No. Each store will need to have its own EOI response. The Main Authorised person (pharmacy owner) will receive an email containing separate personalised links for each one of your stores. Each separate link will take you to a response form for that pharmacy and will contain pre-filled information for that store.

I have read the Australian Government's EOI documentation and these FAQs and still have questions regarding the site requirements I want to ask before I submit my EOI response.ExpandClose
The Australian Government’s EOI document available here and these FAQs outline all the information currently available. Whilst it is appreciated that the more information you can receive on logistics (delivery cycles, vaccine vial volumes, the national booking system etc) the more it will inform your EOI, both the Department of Health and the PPA are unlikely to be able to provide further detail at this stage. We will continue to provide information as it becomes available.
Questions regarding the EOI process can be directed the PPA at support@ppaonline.com.au .

Is access to all the medical equipment listed in the EOI mandatory for my pharmacy to be able to participate? (e.g. defibrillators and Guedel airways)ExpandClose
At this point, while it is not mandatory to have medical equipment on hand or nearby (e.g. defibrillators in shopping centres), your answers to these questions will factor into the assessment of your response. Answering no to these questions will not remove your ability to participate. However, it may mean that you do not participate in the initial roll out.
