The Department of Health and Aged Care (the Department) has confirmed the following extensions to the Medication Management Review (MMR) and Aged Care On-site Pharmacist (ACOP) Measure transition periods.

Medication Management Reviews

The MMR transition period is extended to 31 December 2025. The Department has confirmed there will be no further extensions beyond this date. This provides additional time for MMR credentialed pharmacists to meet the Australian Pharmacy Council (APC) standards by completing an APC accredited MMR recognition of prior learning process or training program.

The following Medication Review Numbers (MRNs) will remain valid until 31 December 2025, or until an updated MRN has been issued as a result of completing an APC accredited MMR recognition of prior learning process or training program, whichever occurs sooner:

  • Australian Association of Consultant Pharmacy (AACP) MRNs that were in effect as of 31 December 2022 (i.e. those pharmacists who had not let their AACP MRN accreditation lapse prior to 31 December 2022);
  • Pharmaceutical Society of Australia (PSA) MRNs;
  • Australasian College of Pharmacy (ACP) MRNs; and
  • Advanced Pharmacy Australia (AdPha – previously known as SHPA) MRNs

Aged Care On-site Pharmacist Measure

The ACOP Measure transition period is extended to 30 June 2026, allowing MMR credentialed pharmacists to participate in the ACOP Measure until this date. MMR credentialed pharmacists participating in the ACOP Measure must complete an APC accredited ACOP training program by 30 June 2026.

From 1 January 2026, all MMR Credentialed Pharmacists who wish to continue claiming the Home Medicines Review (HMR) and Residential Medication Management Review (RMMR) Programs, or participate in the ACOP Measure under transition arrangements, will need to ensure they meet the APC standards for MMR and use the new MRN provided when claiming for these programs.

Please note, as per the current process in the PPA Portal, if a MMR Credentialed Pharmacist transfers to a new training provider and receives a new MRN through that training provider, they will need to use the new number provided when claiming HMR/RMMRs or for participation in the ACOP Measure.
