The Australian Government has confirmed that the Community Pharmacy in Health Care Homes Trial Program will end on 30 June 2021 in line with the ending of the broader Health Care Homes Trial.

The Community Pharmacy in Health Care Homes Trial Program was funded as part of the Sixth Community Pharmacy Agreement (6CPA) to support the incorporation of medication management planning and programs within Health Care Homes. Patients have benefited from patient-centred, coordinated medication management services delivered by their community pharmacy of choice in conjunction with their Health Care Home.

Thank you to all community pharmacies that participated in this significant trial. After the trial ends on 30 June 2021, the trial outcomes will be evaluated and a final report will be published on the Australian Government Department of Health’s website.
