The PPA wishes to advise that there is a new version of the Residential Medication Management Review Patient Consent Form, which you can now access on the PPA website. The new form is also available as a low ink version.

Please note that you must use the new form for every service you provide from 1 July 2026.

What this means for you and your patients

The updated consent form will help you to ensure that patients better understand what a RMMR service entails and how their data will be used. The form also offers you greater flexibility, enabling you to obtain consent in a manner that best suits each patient’s circumstances and needs.

It can be challenging when a patient can give consent but is unable to physically sign the consent form. This updated form allows pharmacists to record verbal consent in these situations, with page 5 of the new consent form outlining the new consent option.

Previously documented consent can still be used

You may have already obtained patient consent for upcoming services and documented this consent on the old version of the consent form. For these services, you do not need to obtain consent again using the updated form.

Using existing printed forms

If you have printed copies of the old form ready to use, you can still use these forms. However, we ask that you adopt the new version of the form as soon as reasonably possible.
